Clinical Clearance Requirements
- Before completing the Clinical Placement Form please check to make sure you have completed all clinical clearance requirements.
- All requirements must be complete to participate in clinical. Follow the instructions below.
Special Note: Some clinical sites may require additional requirements not listed in Canvas. If additional information is needed you will be notified.
- Go to Canvas
- Log on using your NetID and password
- Choose “School of Nursing – Student Information”
- Read the Clinical Clearance Guide “Welcome”
- Click “SON Clinical Clearance Requirements” and choose your degree program
- Click “MODULES” and you will automatically view the requirements for your program.
Please use the clinical clearance grid located on the “syllabus page” in order to verify your program clinical requirements. Students are enrolled in “sections” which correspondence with your program. Please confirm that you are enrolled in the correct section/program
- Click on “GRADES” to see all requirements that you have completed.
Please note that the Office of Clinical Clearance will input your Health and Immunization, Flu Vaccine, Criminal Background Check, Urine Drug Screen and Student Disclosure Information onto Canvas. Please understand that this is an individual manual process for each student and we will update individual records as quickly as possible as the Fall 2017 semester progresses. Students may access this information through the following sources if the information is not available in Canvas:

To submit your health and immunization documents, please log on to the student health services Patient Portal at with your Rutgers NETID and password. To view your immunization records, click on the Immunization tab, View History, and Print. At the bottom of your record, you will find any missing requirements. You may submit any outstanding immunization documentation through the portal under the Upload tab or via fax (973) 972-0018. If you are uncertain about your requirements, you may find your program and corresponding packet at: http://njms.rutgers.edu/shs/immun_son.cfm.

Criminal Background Check, Urine Drug Screen and Student Disclosure Form
Student may log onto CastleBranch and view their status. The criminal background check is good for two years. If you have completed this within the last two years you will not need to do again. Students must check the expiration date of their CBC and Drug Screening and must complete a new background check/drug screening if it is more than two (2) years old.
Students are required to have a current valid American Heart Association "Basic Life Support for Health Care Providers" CPR certification. These certifications can be completed at a variety of training centers and are valid for two (2) years from the day that you pass the course. Students will be asked to upload their CPR card to Canvas. Students can check their status on Canvas
