Dr. Sijia Wei, PhD, RN, PHN, is an Assistant Professor at the Rutgers School of Nursing and core faculty at the Institute for Health, Health Care Policy, and Aging Research. Drawing on her clinical experience as a registered and public health nurse in community-based care settings, Dr. Wei’s research focuses on improving care coordination in care transitions for individuals with multiple chronic conditions and disabilities. She integrates social network analysis with electronic health records, administrative claims data, and other emerging data sources to examine how relationships among clinicians influence patient outcomes, health system performance, and disparities in health outcomes. Her work aims to develop scalable, data-driven approaches to reduce care fragmentation and improve outcomes for high-need populations, including those with Alzheimer’s disease and related dementias. Dr. Wei’s research is supported by the National Institute on Aging through a Pathway to Independence Award (K99/R00). Her work has been published in leading journals across gerontology, nursing, and health services research and reflects a strong commitment to interdisciplinary collaboration and innovation in healthcare delivery.
