New Immunization and Health Record Form
Starting this semester, all students who must provide documentation of immunizations or health record information should submit this information on the newly revised
IMMUNIZATION RECORD & HEALTH CLEARANCE FORM
We will no longer accept individual health reports of any type. All information must be recorded on the required form and submitted to Hurtado Health Center.
If your clinical placement is at risk, please send a copy to the Office of Student Affairs to expedite your clearance.
Forms are available online, at the Office of Student Affairs or 1 Richardson Street.
